Abstract

Astma je kronična upalna bolest dišnih puteva izrazito komplicirane etiologije, karakterizirana varijabilnim i ponavljajućim simptomima. Glavne karakteristike astme su opstrukcija i upala dišnih puteva te bronhalna preosjetljivost. Astma je jedna od najvažnijih kroničnih bolesti na svijetu te je veliki teret za zdravstveni sustav i svjetsku ekonomiju. Samo u Sjedinjenim Američkim Državama, godišnje se na astmu potroši oko 56 milijardi dolara. To je najčešća dječja kronična bolest. Nastanak i razvoj astme te odgovor bolesti na terapiju uvjetovan je brojnim okolišnim i genetskim čimbenicima. Ovisno o pokretaču napadaja astme postoje 2 glavna tipa astme. To su ekstrinzična (alergijska, atopična) i intrinzična (idiosinkratična, neatopična) astma. U patofiziologiji astme sudjeluju brojne vrste stanica imunosnog sustava te strukturne stanice dišnog sustava i medijatori upale koje te stanice stvaraju. Dijagnoza astme temelji se na anamnezi, prisutnosti glavnih simptoma i fizikalnom pregledu te se potvrđuje testovima plućne funkcije. Farmakološka terapija astme uključuje liječenje akutnih egzacerbacija i kontrolu kroničnih simptoma. Prema trenutno prihvaćenim smjernicama, terapija astme je stupnjevita te uključuje 5 koraka. Lijekovi za liječenje astme se mogu podijeliti u dvije skupine. To su brzodjelujući lijekovi koji djeluju kao bronhodilatatori te temeljni lijekovi koji djeluju protuupalno. Većina lijekova za astmu se primjenjuje inhalacijskim putem radi bržeg nastupa djelovanja i manjeg rizika nastanka sistemskih nuspojava. Inhalacijski kortikosteroidi su najvažnija temeljna terapija u svim dobnim skupinama jer su najučinkovitiji u liječenju trajne astme. Sistemski kortikosteroidi se najčešće primjenjuju u terapiji egzacerbacija. Agonisti β2-adrenergičkih receptora dijele se na brzodjelujuće i dugodjelujuće. Salbutamol je glavni lijek za simptomatsku terapiju astme. Dugodjelujući β2-agonisti se uvijek primjenjuju u kombinaciji sa inhalacijskim kortikosteroidima. Antagonisti muskarinskih receptora, modifikatori leukotriena, teofilin i kromoni se uglavnom ne primjenjuju kao samostalna terapija astme već kao dodatna terapija kada se uspješna kontrola bolesti ne može postići inhalacijskim kortikosteroidima. Monoklonska protutijela koriste se za liječenje određenih fenotipova teške refraktorne astme koji ne odgovaraju na terapiju kortikosteroidima i drugim temeljnim lijekovima. Monoklonska protutijela koja se koriste u liječenju astme su omalizumab (anti-IgE) te mepolizumab i reslizumab (anti-IL5).

Abstract (english)

Asthma is a chronic disease of airways. It has very complex etiology and it is characterised by variable and occurring symptoms. Main characteristics of asthma are obstruction and inflammation of airways and bronchial hyperresponsiveness. Asthma is a great economic burden for the healthcare and world economy. Only in the United States, the total cost of asthma to society was $56 billion. It is the most common chronic disease in children. Development of asthma and response of asthma to therapy is influenced by many environmental and genetic factors. Depending on the trigger asthma can be divided into extrinsic (allergic) and intrinsic (non-allergic). The pathophysiology of asthma involves inflammatory and structural cells of the airways and mediators these cells create. Diagnosis of asthma is based on patient anamnesis, physical examination and pulmonary function testing. Therapy of asthma includes treating chronic disease and acute exacerbations. Therapy of chronic asthma is stepwise and includes 5 steps. Drugs for treatment of asthma can be divided in short-acting drugs which are bronchodilators and controller drugs which are anti-inflammatory. Drugs for asthma are usually inhaled because of faster onset of action and lower systemic adverse effects. Inhaled corticosteroids are main controller treatment in all age groups because they are most effective in treating chronic asthma. Systemic corticosteroids mostly used as acute asthma treatment. β2-adrenergic receptor agonists can be short-acting and long-acting. Salbutamol is main drug for symptomatic treatment of asthma. Long-acting β2-adrenergic agonists are always used as add-on treatment with inhaled corticosteroids. Muscarinic receptor antagonists, leukotriene modifiers, theophylline and chromones are rarely used as a singular therapy, they are mostly used as add-on therapy when disease can't be controlled with inhaled corticosteroids alone. Monoclonal antibodies are used for treatment of refractory phenotypes of asthma that can't be treated with corticosteroids and other add-on drugs. Monoclonal antibodies for treatment of asthma are omalizumab (anti-IgE), mepolizumab and reslizumab (anti-IL5).
