Abstract

Glaukom se smatra kroničnom bolesti koja može dovesti do oštećenja optičkog živca i vidnog polja. Ukoliko se ne
liječi ili se liječi nepravilno može dovesti do ireverzibilnog gubitka vida. Budući da ne postoji lijek kojim je moguće
izliječiti glaukom, pacijenti su primorani do kraja života primjenjivati lijekove da bi se prevenirala progresija bolesti i
daljnje oštećenje vida. Jedini način liječenja koji se dosada pokazao učinkovitim u očuvanju vidne funkcije je
snižavanje intraokularnog tlaka (IOT). Svakom bolesniku treba odrediti individualni ciljni tlak pri kojem će progresija
bolesti biti zaustavljena ili odgođena. Preporuka Europskog društva za glaukom je započeti liječenje s monoterapijom,
a ukoliko ona ne bude zadovoljavajuća uvesti kobiniranu terapiju. Najčešće propisivani lijekovi u svijetu, pa tako i u
RH, jesu topikalno primijenjeni analozi prostaglandina i β blokatori.
Ciljevi ovog rada su pregledno i detaljno prikazati farmakoterapijske mogućnosti te naglasiti ulogu ljekarnika u
postizanju željenih terapijskih ishoda. Za svaki antiglaukomski lijek objašnjen je učinak na sniženje IOT-a te su
navedene kontraindikacije i najčešće nuspojave.
Uloga ljekarnika u savjetovanju pacijenta s glaukomom se očituje kroz edukaciju pacijenta o prirodi i tijeku same
bolesti te potrebi za kontinuiranim provođenjem terapije. Kroz detaljno savjetovanje o pravilnoj primjeni
antiglaukomske terapije postiže se veća učinkovitost, a smanjuju rizici od neželjenih učinaka. Poticanje pacijenta na
adherenciju prema terapiji, njegovo aktivno uključivanje u proces liječenja kroz redovite odlaske na kontrolne
preglede, pridržavanje terapijskih smjernica kao i nefarmakoloških mjera naglašavaju važnu ulogu ljekarnika u skrbi
za pacijenta s glaukomom.

Abstract (english)

Glaucoma refers to a group of chronic disorders that can lead to optic nerve damage and visual-field defects. If left
untreated or not treated adequately, it can lead to blindness.While there is no cure patients will need treatment for the
rest of their lives to prevent visual-field deterioration. Currently, the only approach proven to be efficient in preserving
visual function is lowering intraocular pressure (IOP). The target IOP, at which the disease progression is stopped or
delayed, is determined individually in each patient. European Glaucoma Society recommended to initiate the treatment
with monotherapy, but when a single therapy is not sufficient to lower or control the elevated IOP, combination
therapy is indicated. Topical prostaglandin analogs and beta-blockers are the most commonly used medications in the
world, including in Croatia.
The aim of this paper is to provide clear and detailed presentation of pharmacotherapeutic possibilities and to
emphasize the role of pharmacists in achieving the desired therapeutic outcomes. The effect of lowering the IOT was
explained for each antiglaucoma drug. Contraindications and most common side effects are also mentioned.
The role of a pharmacist in counseling a patient with glaucoma is manifested through the patient's education about the
nature and course of the disease itself. Through detailed counseling on proper application of antiglaucoma therapies,
effectiveness is achieved and risks of adverse effects are reduced. Encouraging the patient to adhere to therapy, his
active involvement in the treatment process through regular check-ups, compliance with therapeutic guidelines and
non-pharmacological underline the important role of phramacist in caring for a glaucoma patient.
