Abstract

Cilj istraživanja: Cilj ovog rada bio je analizirati terapijske probleme pacijenata s kardiovaskularnim
bolestima u javnim ljekarnama na temelju podataka prikupljenih tijekom Stručnog osposobljavanja
studenata pete godine Farmaceutsko-biokemijskog fakulteta Sveučilišta u Zagrebu te utvrditi uspješnost
studenata u identifikaciji terapijskih problema. Ispitanici i metode: Studenti su podatke o pacijentima
prikupili u razdoblju od 2016. do 2017. godine, a uključivali su osnovne informacije o pacijentu, povijest
bolesti, propisane lijekove, uočene alergije na lijekove i nuspojave lijekova, procjenu adherencije,
identificirane terapijske probleme te plan ljekarničke skrbi. Za utvrđivanje terapijskih problema korišten je
alat razvijen od Cipolle i sur. (2012). Rezultati: U istraživanje je uključeno 79 studenata koji su prikupili
podatke o 80 pacijenata. Medijan životne dobi pacijenata iznosio je 68 godina (raspon 43-92). Prosječan
broj dijagnoza iznosio je 4,21 ± 2,08 (raspon 1-10), a propisanih lijekova 7,01 ± 3,03 (raspon 2-19). Ukupno
su identificirana 442 terapijska problema, a svaki je pacijent imao barem jedan. Kod najvećeg broja
pacijenata detektirano je pet terapijskih problema (n=13, 16,3%). Studenti su utvrdili 45,9% terapijskih
problema. Najučestaliji uzroci terapijskih problema kardiovaskularnih bolesti bili su nuspojava lijeka (n=53,
21,5%) i preniska doza (n=53, 21,5%). Zaključak: Ovim radom utvrđena je značajna uloga ljekarnika koji
svojim znanjem i kompetencijama mogu pridonijeti optimizaciji terapije pacijenata s kardiovaskularnim
bolestima. Važno je razvijati vještine prepoznavanja terapijskih problema još za vrijeme studija kako bi
budući ljekarnici odgovornim pružanjem ljekarničke skrbi doprinijeli pozitivnim ishodima liječenja.

Abstract (english)

Objectives: The aim of this study was to analyse drug related problems among patients with
cardiovascular diseases in community pharmacy, based on data collected by students in the fifth year of
study at The Faculty of Pharmacy and Biochemistry at the University of Zagreb. The secondary aim was
to assess students' success in identifying drug related problems. Subjects and methods: Students collected
patient data in the period from 2016. to 2017. which included basic patient information, history of illness,
prescribed medications, drug allergies and side effects, adherence, identified drug related problems and
pharmaceutical care plan. Drug therapy problems were evaluated according to categories presented by
Cipolle et al. (2012). Results: 79 students collected data from 80 patients. The median age of the subjects
was 68 years (range 43-92). The average number of diagnoses was 4,21 ± 2,08 (range 1-10) and
prescription drugs 7,01 ± 3,03 (range 2-19). A total of 411 drug related problems were identified and each
patient had at least one drug related problem. Most subjects (n=13, 16,3%) had five drug related problems
in their therapy. Students identified 45,9% of the drug related problems. The most common causes of drug
related problems identified in cardiovascular disease therapy were adverse drug reaction (n=53, 21,5%)
and too low dose of the medicine (n=53, 21,5%). Conclusion: This study indicates a significant role of
pharmacists who can contribute to the therapy optimization of patients with cardiovascular disease. It is
important for future pharmacists to develop their skills to identify drug therapy problems during the study
so that they can prevent them in the future and contribute to positive outcomes of the treatment.
