| Abstract | Nedostatak specifičnoga ranog biokemijskog biljega velik je problem u dijagnostici i liječenju bubrežnih oštećenja,
u prvom redu akutnoga bubrežnog oštećenja (ABO) novorođenčadi. Laboratorijska dijagnostika bubrežnih
oštećenja još uvijek se dobrim dijelom oslanja na promjene koncentracije kreatinina u serumu. Međutim, kreatinin
kao biljeg oštećenja bubrega ima brojne nedostatke, od kojih je najveći nedovoljna dijagnostička osjetljivost.
Posljedica je toga kasna dijagnoza, a samim time i ograničena rana terapijska intervencija. Stoga se kontinuirano
istražuju rani biokemijski biljezi koji bi bili od pomoći u ranom otkrivanju i prevenciji bubrežnih oštećenja.
U ovom istraživanju izmjerene su koncentracije urinskih biljega hepcidina i lipokalina povezanog s neutrofilnom
gelatinazom (NGAL, engl. neutrophil gelatinase associated lipocalin) te su uspoređene s biljezima općeg statusa
bubrežne funkcije koji su trenutačno u upotrebi (kreatinin, cistatin C, eGFR). Radi što boljeg razumijevanja
dijagnostičke točnosti izabranih biljega bubrežne funkcije ispitanici su bili podijeljeni u tri skupine: terminski
zdrava novorođenčad (N-Z, N = 120), novorođenčad s akutnim bubrežnim oštećenjem (N-ABO, N = 20) te skupina
s drugim bubrežnim oštećenjima i bolestima (N-DBB, N = 23). U svrhu ovog istraživanja koristila se isključivo
ostatna količina seruma preostala nakon provedenih rutinskih laboratorijskih pretraga, a uzorci urina uzeti su
unutar 72 sata nakon rođenja.
Rezultati ovog istraživanja utvrdili su veće koncentracije urinskih biljega NGAL-a i hepcidina u novorođenčadi s
ABO u odnosu na zdravu novorođenčad. Koncentracije NGAL-a u pacijenata s drugim bubrežnim bolestima
statistički su značajno veće u odnosu na zdravu novorođenčad, dok kod hepcidina nije opažen bitan porast
koncentracije. Novorođenčad s ABO imala je statistički manje vrijednosti eGFR-a u odnosu na zdravu
novorođenčad primjenom svih triju prediktivnih jednadžbi. Korištenjem cistatin C jednadžbe i CKID jednadžbe
utvrđena je statistički značajna razlika u vrijednostima eGFR-a između skupine s N-ABO i N-DBB. U procjeni
dijagnostičke točnosti za novorođenčad u skupini N-ABO, u ovom se istraživanju NGAL pokazao najboljim
izborom uz vrlo uvjerljive rezultate temeljene na ROC analizi. Dobivena je visoka vrijednost područja ispod
krivulje koja je pri graničnoj vrijednosti 67,3 ng/mL iznosila 0,987, uz osjetljivost 97 % i specifičnost od 93,6 %.
S druge strane, AUC vrijednost za hepcidin iznosila je 0,855, gdje granična vrijednost od 52,1 ng/mL ostvaruje
jako dobru osjetljivost od 81 %, ali znatno slabiju dijagnostičku specifičnost 70 %. Na temelju rezultata ovog
istraživanja u novorođenčadi s ABO rani porast koncentracije urinskih biljega te mogućnost brzoga, jednostavnog,
standardiziranog i automatiziranog mjerenja, u prvom redu NGAL-a, može doprinijeti realnom uključenju ovoga
neinvazivnog biljega u svakodnevnu kliničku praksu. |
| Abstract (english) | The lack of specific early biochemical markers is a major problem in the diagnosis and treatment of kidney damage,
especially acute kidney injury (AKI) in neonates. Laboratory diagnostics of kidney damage still largely rely on
changes in serum creatinine concentration. However, creatinine as a marker of kidney damage has numerous
drawbacks, the biggest of which is inadequate diagnostic sensitivity. As a result, there is delayed diagnosis and
hence limited early therapeutic intervention. Therefore, early biochemical markers that could aid in early detection
and prevention of kidney damage are continuously being researched.
In this study, the concentrations of urinary biomarkers hepcidin and neutrophil gelatinase-associated lipocalin
(NGAL) were measured and compared with commonly used markers of general kidney function (creatinine,
cystatin C, eGFR). To better understand the diagnostic accuracy of the selected kidney function markers,
participants were divided into three groups: term healthy neonates (N-H, N = 120), neonates with acute kidney
injury (N-AKI, N = 20), and a group with other kidney injuries and diseases (N-OKD, N = 23). Only the remaining
serum samples after routine laboratory tests were used for this study, and urine samples were collected within 72
hours after birth.
The results of this study showed higher concentrations of urinary biomarkers NGAL and hepcidin in neonates with
AKI compared to healthy neonates. NGAL concentrations in patients with other kidney diseases were significantly
higher compared to healthy neonates, while no significant increase in hepcidin concentration was observed.
Neonates with AKI had statistically lower eGFR values compared to healthy neonates using all three predictive
equations. Using the cystatin C equation and CKID equation, a statistically significant difference in eGFR values
was found between the N-AKI and N-OKD groups. In the assessment of diagnostic accuracy for newborns in the
N-AKI group, this study showed that urinary NGAL was the best choice with very convincing results based on
ROC analysis. The obtained high area under the curve value was 0,987 at a cutoff value of 67,3 ng/mL, with a
sensitivity of 97% and specificity of 93,6%. On the other hand, the AUC value for hepcidin was 0,855, with a
cutoff value of 52,1 ng/mL achieving good sensitivity of 81%, but significantly lower diagnostic specificity of
70%. Based on the results of this study, the early rise in urinary biomarker concentrations in newborns with AKI
and the possibility of fast, simple, standardized, and automated measurement, primarily of NGAL, can contribute
to the real inclusion of this non-invasive biomarker in everyday clinical practice. |
