Abstract

Usklađivanje terapije (eng. medication reconciliation) je proces uspoređivanja popisa lijekova propisanih od strane liječnika sa svim lijekovima koje pacijent stvarno koristi. Također se može definirati i kao usporedba najbolje moguće medikacijske povijesti (eng. Best Possible Medication History, BPMH) s propisanom terapijom od strane liječnika prilikom prijema, premještanja i otpusta pacijenta iz bolnice te identificiranje i rješavanje svih razlika ili odstupanja (eng. discrepancies). Popis bi, uz receptne lijekove, trebao sadržavati i bezreceptne lijekove, paranteralnu prehranu i dodatke prehrani koji se često zaborave kod prikupljanja medikacijske povijesti.
U istraživanju je uključeno 80 ispitanika prosječne životne dobi od 63 godine(od 20 do 87 godina), koji su bili zaprimljeni hitnim prijemom (73.4%) na Zavod za gastroenterologiju, Klinike za unutarnje bolesti, Kliničke bolnice Dubrava. Utvrđena su 144 odstupanja pod kojih je 43 (29.9%) nenamjerno. Jedno ili više nenamjernih odstupanja pojavilo se kod 26 (32.5%) ispitanika.

Abstract (english)

Medication reconciliation is a process of comparing a list of medicines prescribed by the doctor with all the medicines that the patient really uses. It can also be defined as a comparison of the best possible medication history (BPMH) with prescribed treatment by the doctor when receiving, moving and dismissing the patient from the hospital and identifying and solving all differences or discrepancies. The list, along with prescription drugs, should also include non-prescription drugs, paranteral nutrition and dietary supplements that are often forgotten when collecting medical history.
The study included 80 participant average age of 63 (20-87) who were received by emergency (73.4%) at the Clinical Department of Internal Medicine, University Hospital Dubrava. 144 discrepancies were identified, of which 43 (29.9%) were unintentional. One or more unintentional discrepancies occurred in 26 (32.5%) participants.
