Abstract

Antiagregacijski (antitrombocitni) lijekovi posljednih se godina sve češće koriste u prevenciji i liječenju arterijske tromboze koja se klinički očituje akutnim ishemijskim dogaĎajima. Kontinuirani porast primjene ovih lijekova nametnuo je potrebu za uvoĎenjem agregometrije trombocita u procjeni učinkovitosti liječenja. Iako ove pretrage još uvijek nisu sastavni dio rutinske laboratorijske dijagnostike zbog nedovoljne standardizacije, očita je potreba za njezinim uvoĎenjem u praćenju liječenja ovih bolesnika. Cilj ovog rada bio je ispitati utjecaj antiagregacijskih lijekova na agregabilnost trombocita uz primjenu specifičnih agonista za pojedinu skupinu lijekova (acetilsalicilna kiselina (ASA) i/ili inhibitori ADP receptora) metodom optičke agregometrije na analitičkom sustavu Sysmex CS-2500. Agregabilnost trombocita bila je značajno snižena za obe skupine antiagregacijskih lijekova u odnosu na referentne vrijednosti. U bolesnika kod kojih je antiagregacijska terapija ukinuta zbog operativnog zahvata, oporavak funkcije trombocita postiže se brže u bolesnika liječenih ASA-om, u odnosu na bolesnike liječene inhibitorom ADP receptora ili kombiniranom terapijom.

Abstract (english)

In recent years inhibitors of platelet aggregation (antiplatelet drugs) are increasingly used in the prevention and treatment of arterial thrombosis that is clinically manifested by acute ischemic events. Continued increase in the use of these drugs has imposed the need for introducing platelet aggregometry to assess the efficiency of treatment. Although these assays still are not a part of routine laboratory diagnostics due to insufficient standardization, the need for its introduction in treatment of these patients is apparent. The aim of this study was to investigate the effect of antiplatelet drugs on platelet aggregation using specific agonists for particular drug group (acetylsalicylic acid (ASA) and /or ADP receptor inhibitors) by light transmission aggregometry method on the Sysmex CS 2500 analytical system. Platelet aggregation was significantly reduced for both groups of antiplatelet drugs compared to the reference values. In patients with antiplatelet therapy abolished due to surgery, recovery of platelet function was achieved faster in patients treated with ASA compared to patients treated with inhibitors of ADP receptors or those with combined therapy.
