Abstract

Cilj istraživanja: Cilj ovog rada je pregledno prikazati glavne specifičnosti primjene psihofarmaka u trudnica.
Ispitanici i metode: U tu svrhu pretražena je znanstvena i stručna literatura, relevantni psihijatrijski dijagnostički priručnici i trenutno važeće terapijske smjernice, publikacije stručnih udruga i institucija, baza lijekova Agencije za lijekove i medicinske proizvode te drugi raspoloživi izvori.
Rezultati: Identificirane su i na jednom mjestu pregledno prikazane glavne specifičnosti primjene četiri glavne skupine psihofarmaka – antipsihotika, anksiolitika/hipnotika i sedativa, antidepresiva i stabilizatora raspoloženja u trudnica.
Zaključak: Liječenje psihofarmacima u trudnoći je moguće, a katkad i nužno, međutim potrebno je uvijek uzeti u obzir sigurnost i učinkovitost terapije te potencijalne rizike za majku i dijete. Konačna odluka o primjeni određenog psihofarmaka u trudnoći mora biti donesena na temelju detaljne analize dostupnih podataka te u suradnji s trudnicom i njezinom obitelji, uvijek poštujući pravo trudnice na donošenje odluka o vlastitom liječenju. Valja imati na umu kako je neliječena psihička bolest neovisan čimbenik rizika za nepovoljan ishod trudnoće. Ljekarnici predstavljaju sponu između lijeka, liječnika i pacijenta te mogu pomoći identificirati terapijske probleme, pratiti suradljivost i adherenciju ili prepoznati eventualno pogoršanje bolesti te na vrijeme uputiti pacijenticu liječniku specijalistu psihijatru. Prisutna je potreba za većim sudjelovanjem ljekarnika u skrbi za ovu osjetljivu populaciju čime bi se mogli postići bolji ishodi liječenja.

Abstract (english)

The goal of this research: The goal of this thesis is to present the main aspects of the use of psychopharmaceuticals in pregnant women.
Research methods: A variety of scientific and professional literature, relevant psychiatric diagnosis manuals, publications by the professional associations and institutions, the Agency for Medicinal Products and Medical Devices database, as well as some other available sources were examined.
Results: The main specificities of the use of four main groups of psychopharmaceuticals – antipsychotics, anxiolytics/hypnotics and sedatives, antidepressants and mood stabilizers in pregnant women were identified and presented in one place.
Conclusion: Treatment with psychopharmaceuticals during pregnancy is possible, and sometimes necessary, but however, the safety and efficacy of therapy and the potential risks to mother and child should always be considered. The final decision on the use of a particular psychopharmaceutical in pregnancy must be made on the basis of a thorough analysis of the information available and in cooperation with the pregnant woman and her family, always respecting the pregnant woman's right to make decisions about her own treatment. Untreated psychiatric illness is an independent risk factor for an adverse pregnancy outcome. Pharmacists are the link between the drug, doctor, and patient, and can help identify therapeutic problems, monitor compliance and adherence, or identify possible exacerbations of the disease, and refer the patient to a psychiatrist on time. There is a need for greater involvement of pharmacists in the care for this vulnerable population, thereby achieving better treatment outcomes.
