Abstract

Cilj istraživanja
Cilj ovog specijalističkog rada je dati sveobuhvatan pregled znanstvenih spoznaja o bioaktivnim sastavnicama, mehanizmima djelovanja i dokazanim učincima ekstrakata bodljikave veprine (Ruscus aculeatus L.), da bi se prema znanstveno utemeljenim dokazima mogli donijeti relevantni zaključci o učinkovitosti, kakvoći i sigurnosti njihove primjene u liječenju kronične venske insuficijencije i hemoroida.
Materijal i metode
Istraživanja u okviru ovog specijalističkog rada su teorijskog karaktera te uključuju pregled dostupne literature o predloženoj temi. U pretraživanju su korištene elektronske bibliografske baze podataka Pubmed, Medline i Cochrane Collaboration Library te mrežne stranice relevantnih institucija, kao što je Europska agencija za lijekove. Dodatno su prikazani i proizvodi s bodljikavom veprinom dostupni na hrvatskom tržištu.
Rezultati
Nekliničkim istraživanjima ustanovljeno je antiedematozno i vazokonstrikcijsko djelovanje na vene posredovano stimulacijom α-adrenergičkog sustava. Kliničke studije potvrđuju opravdanost primjene droge Rusci rhizoma u ublažavanju simptoma kronične venske insuficijencije, međutim, uglavnom provedene s kombiniranim pripravcima, pa nema dovoljno dokaza za samostalne učinke bodljikave veprine.
Zaključci
Proizvodi s bodljikavom veprinom u Europskoj uniji se mogu registrirati kao tradicionalni biljni lijekovi na temelju dugotrajne medicinske primjene za ublažavanje simptoma nelagode i težine u nogama uzrokovanih manjim smetnjama venske cirkulacije te za simptomatsko ublažavanje svrbeža i peckanja povezanih s hemoroidima, nakon što liječnik isključi ozbiljna stanja. Ipak se većina proizvoda na tržištu nalazi u kategoriji dodataka prehrani i kozmetičkih proizvoda.

Abstract (english)

The aim of this paper is to provide a comprehensive overview of the scientific knowledge on the bioactive constituents, mechanisms of action and proven effects of butcher's broom extracts (Ruscus aculeatus L.) in order to draw relevant conclusions on the efficacy, quality and safety of their use in the treatment of chronic venous insufficiency and haemorrhoids based on scientifically sound evidence.
Materijal and methods
The research for this paper is theoretical in nature and involves a review of the available literature on the proposed topic. The electronic bibliographic databases Pubmed, Medline and Cochrane Collaboration Library as well as the websites of relevant institutions, such as the European Medicines Agency, were used for the search. In addition, the products available on the Croatian market that contain butcher's broom are presented.
Results
Non-clinical studies have found an anti-edematous and vasoconstrictive effect on the veins mediated by stimulation of the α-adrenergic system. Clinical studies confirm the efficacy of the drug Rusci rhizoma in relieving the symptoms of chronic venous insufficiency, but they were mostly conducted with combination preparations, so there is insufficient evidence for the effect of butcher's broom alone.
Conclusion
Butcher's broom products may be registered in the European Union as traditional herbal medicinal products exclusively on the basis of long-term use for the relief of discomfort and heaviness in the legs caused by mild disorders of venous circulation and for the symptomatic relief of itching and burning associated with haemorrhoids, after serious conditions have been ruled out by a medical doctor. However, most of the products on the market are in the category of food supplements and cosmetics.
