Abstract

Atopijski dermatitis je kronična upalna dermatoza koja pogađa 15 do 30% djece te 1 do 3 % odraslih. Većini oboljelih simptomi se povlače do razdoblja adolescencije, ali bolest može progredirati u astmu ili alergijski rinitis. Bolest je karakterizirana kompleksnom patogenezom koja uključuje kombinaciju genetičkih, imunosnih i okolišnih čimbenika koji posljedično dovode do narušavanja barijerne funkcije kože i neravnoteže imunosnog sustava. Najčešći simptomi su svrbež, crvenilo, edem, suhoća kože, erozije te lihenifikacija. U prevenciji je važna hidratacija kože te izbjegavanje čimbenika koji dovode do pogoršanja. Osnovu terapije čine topikalno primjenjeni kortikosteroidi, a na određenim lokacija preporučuju se lokalni inhibitori kalcineurina. Kod težih oblika bolesti primjenjuju se sistemski imunomodulatori, animikrobni lijekovi, kortikosteroidi i retinoidi. Novi terapijski pristupi su usmjereni na inhibiciju Th2 imunosnog odgovora i redukciju upale te su trenutno u različitim fazama kliničkih ispitivanja.

Abstract (english)

Atopic dermatitis is a chronic inflammatory dermatosis that affects 15 to 30% of children and 1% to 3% of adults. The majority of affected individuals have a resolution of disease by adulthood but it may progress to asthma or allergic rhinitis. The disease is characterized by a complex pathogenesis involving genetic, immunologic and environmental factors which lead to a dysfunctional skin barrier and dysregulation of the immune system. The symptoms often include pruritus, erythema, edema, xerosis, erosions and lichenification. Prevention is focused on skin hydration and avoidance of provocation factors. Topical corticosteroids are the mainstay of therapy and are used for exacerbation management and proactive therapy in selected cases. In certain locations, topical calcineurin inhibitors such as pimecrolimus and tacrolimus are preferred. Systemic therapy options such as immunomodulators, corticosteroids, antimicrobial agents and retinoids are used for severe cases. New therapeutic options are focused on inhibiting Th2 immune response and reduction of inflammation and are currently in different phases of clinical trials.
